Ischemic Stroke – Landscape & Forecast – Disease Landscape & Forecast (G7)

Acute ischemic stroke (AIS) affects millions of individuals across G7 countries, according to Clarivate Epidemiology’s tracked lifetime and 12-month prevalent cohorts. As a time-critical emergency, AIS necessitates rapid intervention to preserve salvageable brain tissue. The standard of care includes IV thrombolysis within 4.5 hours and mechanical thrombectomy for eligible large-vessel occlusions. Roche / Genentech’s tenecteplase has expanded reperfusion options, offering a longer half-life and single-bolus administration compared with alteplase. Despite this advance, further innovation in the management of AIS is sorely needed to bring neuroprotective/restorative/reparative and other new therapies to AIS patients to optimize their functional recovery. However, few strategies in this arena have achieved clear clinical proof-of-concept, and trial failures have been the historical norm. Nonetheless, the AIS pipeline is large, active, and diverse regarding mechanisms of action, and the field is focused on advancing trial design.
