Acute coronary syndrome (ACS) is a leading cause of morbidity and mortality in developed countries. Despite aggressive management of atherothrombotic risk factors through LDL-C lowering, blood-pressure control, and antithrombotic therapy, post-ACS patients face a lifelong risk of recurrent ischemic events. This persistent residual risk is driving the expansion of the long-term ACS secondary prevention market, with innovation increasingly focused on more intensive lipid management and biomarker-defined treatment of residual risk. Market growth will be driven primarily by continued intensification beyond statins and ezetimibe, including broader use of PCSK9 inhibitors, and selective use of additional oral LDL-C-lowering agents such as bempedoic acid and obicetrapib. The market will also expand through novel mechanisms that address residual risk beyond traditional factors. Lp(a) inhibitors and ziltivekimab will create new add-on opportunities by targeting elevated Lp(a) and residual inflammatory risk, respectively. Antithrombotics are not covered in this report.
