Ankylosing spondylitis (AS) is a chronic inflammatory disease that primarily affects the sacroiliac joints and the axial skeleton, leading to persistent back pain and stiffness. The main treatment goals include alleviating the signs and symptoms, improving physical function, preventing disability, and slowing or preventing structural damage. TNF-alpha inhibitors such as AbbVie’s Humira, Amgen’s Enbrel, Johnson & Johnson Innovative Medicine’s Remicade and Simponi / Simponi Aria, UCB’s Cimzia, can effectively reduce disease activity in AS, particularly in patients who require potent therapies beyond first-line NSAIDs. The IL-17 inhibitors, Novartis’s Cosentyx, Eli Lilly’s Taltz, and UCB’s Bimzelx, offer an alternative MOA to TNF inhibition. The novel JAK inhibitor class comprises Pfizer’s Xeljanz and AbbVie’s Rinvoq, which are the only nonbiological targeted therapies for AS.
