Colorectal Cancer – Landscape & Forecast – Disease Landscape & Forecast (G7)

Doublet or triplet chemotherapy, alone or in combination with angiogenesis inhibitors, remains the cornerstone of treatment for metastatic colorectal cancer. Patients without RAS or BRAF mutations also benefit from EGFR inhibitors (Merck & Co. / Eli Lilly’s Erbitux and Amgen’s Vectibix), which are recommended for first-line treatment of patients with left-sided tumors. Additional targeted therapies have expanded treatment options for specific molecular subgroups, including immune checkpoint inhibitors (Merck & Co.’s Keytruda and Bristol Myers Squibb’s Opdivo and Yervoy) for MSI-high and/or dMMR tumors, BRAF inhibitors (Array BioPharma / Pfizer’s Braftovi) for BRAF V600E-mutated disease, KRAS G12C inhibitors (Bristol Myers Squibb’s Krazati and Amgen´s Lumakras) for KRAS G12C-mutated tumors, and HER2-targeting agents (Chugai’s Perjeta and trastuzumab and Seagen’s Tukysa), available in some markets for HER2-positive metastatic disease.

Late-phase development programs will further broaden the treatment landscape both in early-stage and metastatic colorectal cancer. In early-stage colon and rectal cancers, immune checkpoint inhibitors aim to expand the role of immunotherapy beyond the metastatic setting for MSI-high/dMMR tumors. In metastatic disease, key areas of investigation include therapies targeting MET and CEACAM5 expression, next-generation KRAS G12C inhibitors for KRAS G12C-mutated tumors, novel EGFR-directed therapies for RAS/BRAF wild-type disease, and immunotherapy-based approaches for MSS/pMMR disease.
