Migraine – Current Treatment – Treatment Algorithms: Claims Data Analysis – Acute Migraine (US)

Episodic and chronic migraines are often disabling and reduce patients’ quality of life. Despite triptans’ dominance in the treatment algorithm, many patients respond inadequately, cannot tolerate them, or have cardiovascular contraindications—creating significant unmet need and opportunity for alternative acute treatments. Since 2020, the acute market has diversified beyond vasoconstrictors, led by oral calcitonin gene-related peptide (CGRP) receptor antagonists (gepants) such as AbbVie’s Ubrelvy, Pfizer’s Nurtec ODT, and intranasal zavegepant, expanding options for patients needing rapid, non-oral relief. Competition for fastacting acute treatments has intensified with reformulations of established agents, including Satsuma’s Atzumi (DHE nasal powder) and Amneal’s Brekiya (the first DHE autoinjector), while Axsome Therapeutics’ Symbravo (meloxicam / rizatriptan) targets prior acute / gepant nonresponders. Our quantitative analysis of national patient-level claims data examines the growing uptake of new brands to treat both newly diagnosed and recently treated patients in a market characterized by numerous generic options.

Questions answered

  •  What patient shares do key acute therapies capture by line of therapy in newly diagnosed migraine patients? What are the quarterly trends in prescribing among newly diagnosed and recently treated patients?
  • How have oral CGRP receptor antagonists been integrated into the acute treatment algorithm, and what is their source of business?
  • What percentage of migraine patients initiate acute drug therapy within two years of diagnosis, and how quickly? What percentage of patients progress to later lines of acute treatments within the same period?
  • What are the product-level compliance and persistency rates among drug-treated patients?
