Uptake of newer, innovative but higher-priced devices has therefore been somewhat slow.<\/p>\n<p>However, manufacturers, third-party digital health companies and payers still aim to provide better solutions to reduce patient burden, enhance management adherence and improve patient lives. Novel devices that provide superior ease-of-use and simplify diabetes management appeal to patients, while those that improve treatment compliance and reduce costly complications appeal to payers, even when they come at a premium price.<\/p>\n<p>For example, a number of competitors have recently introduced pre-calibrated continuous glucose monitoring (CGM) devices that eliminate the need for daily finger prick calibrations. The closed-loop system combines both CGM and insulin delivery technology in a single device, responding to the patient\u2019s insulin needs in real time by monitoring glucose levels and automatically adjusting insulin delivery. Although adoption was initially hindered by its high cost, increased coverage by insurance companies improved its affordability. Since then, Medtronic has developed additional models within the family and launched its next-generation hybrid closed-loop <strong>Mini Med 780G<\/strong> in the summer of 2020 with CE marking in Europe.<\/p>\n<p>In December 2019, <a href=\"https:\/\/investor.tandemdiabetes.com\/news-releases\/news-release-details\/tandem-diabetes-care-announces-fda-clearance-tslim-x2-insulin#:~:text=(NASDAQ%3A%20TNDM)%2C%20a,1%2C%20and%20the%20first%20system\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>Tandem Diabetes Care\u2019s t:slim X2<\/strong><\/a> insulin pump with Control-IQ advanced hybrid closed-loop technology was approved for use by the FDA. This represented the first interoperable automated glycemic controller device approved by the FDA and paved the way for integrated CGMs and alternate controller-enabled (ACE) insulin pumps to be used with interoperable automated glycemic controllers as a complete automated insulin dosing (AID) system. When launched, the integration of the t:slim X2 with <strong>Dexcom&#8217;s G6<\/strong> CGM enabled automated correction boluses within an AID system.<\/p>\n<p>Other strategic partnerships aimed at pairing insulin delivery technology with CGMs for a closed-loop AID system include:<\/p>\n<ul>\n<li><strong>Insulet\u2019s Omnipod DASH<\/strong> and <strong>Horizon<\/strong> closed-loop tubeless insulin pump systems with the <strong>Dexcom G6 and G7<\/strong> (expected launch in early 2021) CGMs and the Omnipod Horizon with the <strong>Abbott Freestyle Libre<\/strong> CGM (Omnipod Horizon is slated for an early 2021 launch in the U.S.)<\/li>\n<li><strong>Beta Bionics\u2019 iLet<\/strong> insulin pump (slated for 2021 launch in the U.S.) with the <strong>Dexcom G6<\/strong> CGM or <strong>Senseonics Eversense<\/strong> CGM<\/li>\n<li><strong>Bigfoot Biomedical\u2019s Unity<\/strong> \u201csmart\u201d insulin pen (submitted to the FDA in December 2020) with the <strong>Abbott Freestyle Libre<\/strong> CGM<\/li>\n<li><strong>Tandem Diabetes Care\u2019s t:slim X2<\/strong> insulin pump with the <strong>Abbott Freestyle Libre<\/strong> CGM<\/li>\n<li><strong>Novo Nordisk\u2019s NovoPen 6<\/strong> and <strong>NovoPen Echo Plus<\/strong> (delayed launch for both) \u201cconnected\u201d insulin pens with the <strong>Dexcom G6<\/strong> and <strong>Abbott FreeStyle Libre<\/strong> CGMs<\/li>\n<li>Agreement for exclusive rights for Eli Lilly to commercialize <strong>Ypsomed\u2019s YpsoPump<\/strong> in the U.S. once approved by the FDA, in preparation for a smartphone-based AID system combining the YpsoPump and <strong>Dexcom CGM<\/strong> for automated insulin dosing<\/li>\n<\/ul>\n<p>To expedite time to market of their insulin delivery technologies, these companies have opted for partnerships rather than developing their own CGM devices. From the CGM device company standpoint, such partnerships allow them to continue generating device sales as closed-loop systems continue to gain popularity.<\/p>\n<p>&nbsp;<\/p>\n<h3>Collaborations connect diabetes care device manufacturers with technology firms to facilitate digital diabetes management<\/h3>\n<p>Mobile technology and advanced analytics have penetrated many industries, and diabetes care is no exception. Partnerships between diabetes care device manufacturers and technology firms enable manufacturers to improve their existing glucose monitoring and insulin delivery devices by taking advantage of mobile phone applications and miniaturized electronic analytics. Patient benefits include improved convenience, self-management capabilities and treatment compliance in addition to fewer support issues, while healthcare providers have enhanced access to patient data for improved care coordination.<\/p>\n<p>The <a href=\"https:\/\/www.mobihealthnews.com\/content\/medtronic-ibm-watson-launch-sugariq-diabetes-assistant\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>Sugar.IQ<\/strong> app resulted from a partnership between Medtronic and IBM<\/a>, integrating IBM Watson\u2019s cognitive computing capabilities with Medtronic\u2019s CGM to enable better diabetes management by patients through access to real-time, personalized insights within a mobile app.<\/p>\n<p><a href=\"https:\/\/onduo.com\/virtualclinic\/\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>Onduo Virtual Diabetes Clinic<\/strong><\/a> is a program provided by Verily, Google\u2019s life science-focused sibling company. Intended to provide patient support for diabetes management, the program includes a digital health platform (mobile app), tracking tools such as a CGM (Dexcom is the preferred CGM device supplier), personalize guidance using predictive analytics and a virtual clinic. Since its inception in 2016, its coverage has extended to all 50 states, and clinical trial data have been published showing an average 2.3% reduction in HbA1c levels for patients with baseline HbA1c &gt;9.0% in a <a href=\"https:\/\/journals.sagepub.com\/doi\/10.1177\/1932296819888662\" target=\"_blank\" rel=\"noopener noreferrer\">preliminary study<\/a>, supported by similar findings (2.4% reduction in HbA1c for patients with baseline HbA1c &gt;9.0%) in a <a href=\"https:\/\/www.jmir.org\/2020\/8\/e21778\/\" target=\"_blank\" rel=\"noopener noreferrer\">follow-up study<\/a>. In addition, Onduo partnered with life insurer John Hancock in its <strong>Aspire program<\/strong> for people with diabetes, providing savings on life insurance premiums and rewards for achieving and maintaining target HbA1c levels.<\/p>\n<p>The <a href=\"https:\/\/www.fiercehealthcare.com\/finance\/teladoc-finalizes-blockbuster-deal-livongo-less-than-three-months#:~:text=Telehealth%20giant%20Teladoc%20completed%20its,Livongo%2C%20the%20company%20announced%20Friday.&amp;text=Under%20the%20terms%20of%20the,%2411.33%20for%20each%20Livongo%20share.\" target=\"_blank\" rel=\"noopener noreferrer\">recent merger between Livongo and Teladoc<\/a> combines telemedicine with <strong>Livongo\u2019s platform for managing chronic conditions<\/strong>, such as diabetes, that provides an application interface, personalized tips and coaching. Livongo had previously established partnerships with Dexcom to integrate Dexcom\u2019s CGM into the platform and worked with plans such as Kaiser Permanente and BlueCross BlueShield of Kansas City to provide their service at no cost to plan members.<\/p>\n<p>Numerous other partnerships have occurred in this space to provide more patient options, such as:<\/p>\n<ul>\n<li><strong>Novo Nordisk\u2019s NovoPen 6<\/strong> and <strong>NovoPen Echo Plus<\/strong> insulin pens are planned to connect with <strong>Roche\u2019s mySugr<\/strong> app and <strong>Accu-Chek Smart Pix<\/strong> device reader as well as the <strong>Glooko Diasend<\/strong> diabetes management platform<\/li>\n<li data-leveltext=\"\uf0b7\" data-font=\"Symbol\" data-listid=\"12\" data-aria-posinset=\"2\" data-aria-level=\"1\"><span data-contrast=\"none\">Eli <\/span><span data-contrast=\"none\">Lilly\u2019<\/span><span data-contrast=\"none\">s <\/span><span data-contrast=\"none\">personalized diabetes management system, which is currently in development, will integrate products from <\/span><span data-contrast=\"none\">Dexcom<\/span><span data-ccp-props=\"{&quot;134233279&quot;:true,&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/li>\n<li>The <strong>Tidepool Loop<\/strong> mobile app (submitted to the FDA in December 2020), which was developed by an open-source data nonprofit, is designed to work with integrated CGMs, such as the <strong>Dexcom G6<\/strong>, and ACE insulin pumps, such as <strong>Tandem Diabetes Care\u2019s t:slim X2<\/strong> and <strong>Insulet\u2019s Omnipod DASH<\/strong><\/li>\n<li><strong>Glytec\u2019s Glucommander<\/strong>, a software-as-medical-device (SaMD) platform, was recently tested in combination with <strong>Abbott\u2019s Freestyle Libre<\/strong> CGM in a <a href=\"https:\/\/plan.core-apps.com\/tristar_ada20\/abstract\/0afe66a1-b4c6-4fdc-9ca4-db834e0fcd53\" target=\"_blank\" rel=\"noopener noreferrer\">prospective study<\/a> with 25 adults with type 2 diabetes and HbA1c &gt;8%. Time in range significantly increased from 48% to 74% over the 4-week study period.<\/li>\n<li>Omada, a digital health startup with a digital diabetes prevention program, recently released positive results from a <a href=\"https:\/\/www.omadahealth.com\/press\/press-release-omada-digital-diabetes-prevention-program-shows-sustained-hba1c-reduction-and-weight-loss-in-randomized-control-trial\" target=\"_blank\" rel=\"noopener noreferrer\">599-person clinical trial<\/a> showing the program\u2019s effectiveness, including reduced HbA1c levels and weight.<\/li>\n<\/ul>\n<p>Studies like these are likely to become more common as companies aim to show evidence for the efficacy of their devices and platforms on patient outcomes.<\/p>\n<p>&nbsp;<\/p>\n<h3>Companies that expand coverage of their devices will reach more patients and be at a competitive advantage<\/h3>\n<p>Although only patients with type 1 diabetes have historically received full reimbursement for more expensive diabetes care devices such as CGM devices, a number of companies have recently sought to expand this coverage to include the large pool of patients with type 2 diabetes. Because most of patients with diabetes have type 2 diabetes, gaining coverage for these patients should improve disease management and patient quality of life, decrease the healthcare costs associated with poor management and increase device sales.<\/p>\n<p>In 2017, Dexcom\u2019s G5 CGM device became <a href=\"https:\/\/www.dexcom.com\/news\/medicare-announces-criteria-covering-dexcom-g5-mobile-cgm-for-all-people-with-diabetes-on-intensive-insulin\" target=\"_blank\" rel=\"noopener noreferrer\">the first to be covered<\/a> for patients with type 1 or type 2 diabetes managing their condition with insulin. Abbott Laboratories also gained Medicare coverage soon after FDA approval for non-adjunctive use of its CGM devices, which is required for the Centers for Medicare and Medicaid Services (CMS) to consider covering CGM devices. However, as companies such as Onduo and Omada continue to show efficacy and improved outcomes through clinical trials, the long-term cost benefit and ability to outweigh up-front costs will become more obvious to payers.<\/p>\n<p>&nbsp;<\/p>\n<h3>COVID-19 has impacted patient care, accelerating device integration and acceptance<\/h3>\n<p>Disruptions to daily life and healthcare access during the COVID-19 pandemic have resulted in suboptimal care for many people, including those with type 2 diabetes. In a <a href=\"https:\/\/decisionresourcesgroup.com\/report\/cchxus0032-2020-digital-cybercitizen-health-u-s-2020\/\" target=\"_blank\" rel=\"noopener noreferrer\">recent Clarivate survey<\/a> conducted with adults in the U.S., 44% had experienced a change in healthcare routine as a result of COVID-19; the impact was greater for adults with type 2 diabetes, with 65% reporting a change. These respondents with type 2 diabetes:<\/p>\n<ul>\n<li>Experienced disruptions in doctor appointments (42%)<\/li>\n<li>Had delayed treatments (22%)<\/li>\n<li>Were non-adherent to treatment (41%)<\/li>\n<li>Missed prescription refills (23%)<\/li>\n<li>Had their care shifted to digital delivery (e.g., remote monitoring, virtual consults; 32%).<\/li>\n<\/ul>\n<p>Because of these challenges in accessing care, 33% of respondents with type 2 diabetes felt that the importance for digital support tools that help with health management, particularly wearable devices (18%), had increased due to the pandemic.<\/p>\n<p>&nbsp;<\/p>\n<h3>Innovations in diabetes management devices and platforms are likely to continue<\/h3>\n<p>Innovations in digital diabetes management provide greater options for patients to manage their own care, which is attractive to patients, providers and payers alike. The increased patient demand for telemedicine and remote healthcare monitoring in the context of COVID-19 has changed the fabric of healthcare provision and highlighted the convenience and greater patient reach associated with devices and patient-centered platforms.
